Read these clauses clearly before attempting to sign the Application Form -

  1. This Completed Application is to the best of my knowledge and understanding and I agree to ask the agency representatives any particular term or explanation that is confusing to me, over and over again, until I am fully clear. I also agree to not submit any information derogatory to my interest or beliefs or those that may be deemed as harmful to my interests.
  2. Before going in for any interview or commencing work at any company through CERTX, I shall be fully clear of the terms of Payment and Benefits if any.
  3. I also agree to submit my Social Insurance Card No. if I am selected for a particular position, to CERTX and shall fully understand the terms of payment as well as regulations governing the same under the instructions/auspices of HRDC.
  4. I hereby agree to be a responsible worker at all times and shall provide CERTX with information regarding my temporary leave of absence from work or permanent discontinuance if I do not intend to go to that particular work. In this regard I shall give CERTX prior notice of at least 24 hours. Under emergent situations where a prior notice is not possible I must provide CERTX with important valid documentation e.g. Doctors note. If such a process is not maintained or followed by me, CERTX has the full right to penalize me adequately and/or shall terminate me from any future work and/or decline any future reference.
  5. By filling out this application I agree to not directly pursue the company I have been denoted for by CERTX, and shall refrain from trying to achieve employment status directly with the company, by communicating directly or through any secondary agent, both before or after my interview has been conducted at that company by the concerned representative, other than the interview at CERTX.
